But how do you register? What do you need to prepare? And what happens when you visit your chosen YAKAP Clinic?

Here is a simple, step-by-step guide based on the latest PhilHealth guidelines.

What is PhilHealth YAKAP?

The Yaman ng Kalusugan (YAKAP) is PhilHealth’s primary care program designed to help members access comprehensive and continuing primary healthcare.

Through YAKAP, members can access services that may include medical consultations, essential laboratory and diagnostic services, medicines under the PhilHealth GAMOT benefit, and selected outpatient cancer screening services, subject to applicable PhilHealth rules and eligibility requirements.

More than simply providing a one-time checkup, YAKAP aims to establish a continuing relationship between the beneficiary and their chosen YAKAP Clinic.

Who Can Avail of YAKAP?

Eligible PhilHealth members and beneficiaries may select and become empaneled with an accredited YAKAP Clinic.

Before proceeding, it is important to make sure that your PhilHealth membership information is properly registered and that your PhilHealth Identification Number (PIN) is available.

PhilHealth currently maintains an official list of accredited healthcare facilities, including YAKAP Clinics.

What Do You Need Before Registering?

To make the process easier, prepare your PhilHealth information before going to your chosen YAKAP Clinic.

Basic information to prepare:

• PhilHealth Identification Number (PIN)
• Updated PhilHealth membership information
• A valid ID, when required for identity verification
• Other supporting documents, when applicable to your membership or beneficiary status

PhilHealth’s current process uses digital identity verification through the PhilHealth Check Utility (PCU). In certain situations, such as when a beneficiary does not have a PhilSys registration record, a valid ID may be required by the clinic for verification.

Important: Specific documentary requirements may depend on your individual PhilHealth records and circumstances. It is best to contact your chosen YAKAP Clinic before your visit if you are unsure about what to bring.

Step-by-Step Guide to PhilHealth YAKAP

STEP 1: Check Your PhilHealth Membership Information

Make sure you have your PhilHealth Identification Number (PIN) and that your membership information is properly registered.

PhilHealth’s Member Portal allows members to access their PhilHealth records, contributions, MDR, and other online services. It also provides the option to select a YAKAP Clinic.

If you need assistance with your membership record, you may also visit a PhilHealth Local Health Insurance Office (LHIO).

STEP 2: Choose Your YAKAP Clinic

Once your PhilHealth information is ready, select your preferred accredited YAKAP Clinic.

PhilHealth provides digital options for clinic selection, including the PhilHealth Member Portal and the eGovPH Mobile App. Members who need assistance may also seek help through a PhilHealth office or through an appropriate clinic-assisted process.

Choose a clinic that you can conveniently visit because YAKAP is intended to establish an ongoing primary care relationship.

STEP 3: Visit Your Chosen YAKAP Clinic

After selecting your clinic, visit the facility for your initial YAKAP encounter.

If you have not yet selected a YAKAP Clinic, the clinic may assist you with the appropriate selection process, depending on the available system and applicable PhilHealth procedures.

Bring your PhilHealth information and identification documents that may be needed for verification.

STEP 4: Undergo the PhilHealth Check Utility (PCU) Liveness Check

Before proceeding with the YAKAP encounter, you will undergo an identity verification and liveness check through the PhilHealth Check Utility (PCU).

The PCU uses information such as your name, date of birth, sex, and facial biometrics for identity verification, subject to PhilHealth’s data privacy policies.

A successful PCU liveness check generates a PCU transaction number, which forms part of the YAKAP empanelment process.

STEP 5: Complete Your First Patient Encounter (FPE)

The next step is the First Patient Encounter (FPE).

The FPE allows the YAKAP Clinic to establish your initial health information and patient record.

PhilHealth’s YAKAP process requires the FPE to be completed and the corresponding FPE data to be available in the system before proceeding with the digital YAKAP Empanelment Slip.

Note: The FPE is part of the YAKAP registration and empanelment process and should not automatically be treated as the same thing as a regular consultation.

STEP 6: Proceed to the Digital YAKAP Empanelment Slip (YES)

This is an important update under the latest PhilHealth guidelines.

The YAKAP Empanelment Slip (YES) is mandatory for YAKAP beneficiary empanelment.

As of the latest PhilHealth guidelines, the Digital YES is the primary and required platform for YAKAP beneficiary empanelment. It is processed through the Health Care Institution (HCI) Portal by the YAKAP Clinic.

The digital process proceeds after the system confirms the beneficiary’s FPE.

STEP 7: Review the YES Terms and Patient Rights

Before completing the YES, the YAKAP provider is required to explain the terms and conditions of the YES, including the patient’s rights.

The beneficiary then reviews and agrees to the applicable terms.

This ensures that the patient understands the continuing care relationship with the selected YAKAP Clinic.

STEP 8: Complete the Digital Consent and Signature

After agreeing to the YES terms, the beneficiary undergoes another PCU liveness check.

Under the current digital YES process, this second PCU liveness check serves as the beneficiary’s digital signature.

The corresponding PCU transaction number is entered into the system to complete the digital empanelment process.

STEP 9: Complete Your YAKAP Empanelment

Once the required digital process has been completed, your empanelment with the selected YAKAP Clinic is established.

The goal is to have a continuing healthcare relationship with your chosen primary care provider rather than simply visiting a clinic for a single transaction.

PhilHealth’s current YAKAP system is designed to support continuous primary care and health monitoring.

What About the Printed or Manual YES?

The latest PhilHealth Advisory No. 2026-0052, dated September 29, 2026, emphasizes the mandatory use of the Digital YAKAP Empanelment Slip (YES).

A printed copy of the YES may still be provided to the beneficiary upon request. Wet signatures may also be used for documentation purposes, but they do not replace the required digital process.

PhilHealth also states that manual or printed YES submissions require additional verification and approval and may therefore take longer to process.

These may include primary care consultations, laboratory and diagnostic services, medicines under the GAMOT benefit, and selected cancer screening services.
